The Landmark Heart Center, located at 186 Cass Avenue (across from the Woonsocket Unit), is dedicated to preventing disease through exercise, education, and supervision by specialists.

The Cardiac Rehabilitation Program ("Cardiac Rehab") is a specialized program for patients with heart conditions, such as heart attack, angina, or recent cardiac surgery/procedure (stents). This program is certified by the American Association of Cardiovascular and Pulmonary Rehabilitation (AACVPR).
Our goal with this program is to increase your overall functional capacity and build up cardiovascular efficiency. You are closely supervised by a team of cardiac rehabilitation nurse specialists, exercise specialists, and dietitians.
The length and extent of your involvement is this program is based on the severity of your existing cardiac condition. During exercise, patients are closely supervised by staff through assessment and telemetry monitoring.
Our Heart Center staff maintains close communication with your referring physician in order to maximize medical management and quickly handle any complications that may arise. We also offer educational seminars designed to aid in lifestyle modifications and risk reduction, enabling patients to better manage their cardiac disease.
You qualify for Cardiac Rehab if you have recently had one or more of the following:
Once you are cleared by your physician, cardiac rehabilitation can begin. All education and exercise takes place in the specially designed Cardiac Rehabilitation Center. Upon completion of the formal Cardiac Rehab Program, participation in the Cardiac Rehabilitation Maintenance Program is strongly advised. This program is also available at the Heart Center.
Physician referral is required. Costs are covered by most major insurance carriers if you enroll within six months to one year of your heart attack or cardiac event/procedure. Co-payments and deductibles may apply. Please check with your insurance provider.
The Cardiac Rehabilitation Maintenance Program is an ongoing program for people who have completed a formal cardiac rehabilitation program or have a history of a heart condition.
Benefits of the program include:
Self-referral. Physician clearance and a yearly stress test are required. Self-pay monthly fee.
The Exercise to Health Program is intended for people who want to maintain good health by beginning an exercise program, but do not know how to get started. Credentialed Heart Center staff can develop individualized programs targeting such specialized goals as muscle strengthening, weight loss, increased endurance, lifestyle modifications and risk reduction.
Self-referral. After an initial screening, clients may be requested to undergo additional testing and to obtain physician clearance.
The Diabetic Outpatient Education Program is a six-week group educational series designed to assist diabetic patients in improving blood glucose management. The program also aims to improve quality of life and control the damaging effects of poor diabetic management on body systems.
The Heart Center also offers individual sessions for newly diagnosed diabetics, women with gestational diabetes, and diabetics having management problems or needing insulin or glucose meter instruction.
The educational sessions conducted at the Landmark Heart Center by certified diabetes nurses, dietitians and pharmacist. The program is certified by the American Diabetes Association.

The Heart Center offers a Diabetes Support Group that meets the third Tuesday of every month from 10 a.m. to 11 a.m. The support group features occasional speakers and educational material, as well as emotional support and coping skills/
